Our Mission ~ Children with special needs face the challenge of reaching their potential across educational and health domains. Their success relies on partnerships between families, teachers, health care professionals, and community partners to coordinate and plan care with family members to assure all factors impacting progress are addressed. Unfortunately, many communities experience a gap between education and health care. In a time when chronic illnesses are increasing in children at alarming rates, this gap must close. This conference has the mission of bringing together health care professionals and educators to network, problem solve, collaborate and coordinate care for children and youth with special needs and their families.
